鼻内镜手术治疗不同年龄鼻窦炎患者的临床研究

【摘要】 目的 比较鼻内镜手术治疗中青年和老年鼻窦炎患者的临床疗效。方法 将自2008年2月至2010年2月来我科就诊的92鼻窦炎患者按照年龄分为两组,其中老年组48例,中青年组44例,两组患者均行鼻内镜下手术治疗,术后随访6~12个月,比较两组的临床疗效及术后并发症发生情况。结果 鼻内镜手术治疗老年患者同样有满意的疗效,并发症少,和中青年组比较差异无统计学意义(P＞0.05,P＞0.05)。结论 鼻内镜手术治疗老年鼻窦炎患者同样具有很好的疗效,值得推广。

【Abstract】 Objective To compare the treatment effect of endoscopic sinus surgery in middle-aged and elderly patients with sinusitis.Methods 82 patients with sinusitis in our department since February 2008~February 2010 were divided into two groups:48cases in the elderly group and 44 cases in the middle-aged group were underwent endoscopic sinus surgery treatment and were followed up for 6~12 months, then the clinical efficacy and postoperative complications between the two groups were compared.Results The treatment effect of endoscopic sinus surgery in elderly patients with sinusitis was excellent too, and the rate of postoperative complications was lower, the difference compared with the middle-aged group was not statistically significant(P＞ 0.05, P＞ 0.05).Conclusion Endoscopic sinus surgery in treatment of elderly patients with sinusitis has excellent effect too and it is worth to promote.

鼻窦炎耳鼻喉科的一种常见疾病,患者主要临床表现可有鼻阻、头痛、流涕嗅觉减退等,给患者的生活带来了严重的影响。对于鼻窦炎的发病机制目前还不清楚,多数人认为可能和机体自身因素和外界环境的多种因素如感染、变态反应性炎症及空气污染等有关。目前主要应用鼻内镜手术治疗慢性鼻窦炎,特别是对保守治疗或传统手术治疗无效的慢性鼻窦炎有较好的疗效,另外该种手术方式也为老年鼻窦炎患者的治疗提供了新的方法[1,2],为了探讨鼻内镜手术在老年患者中的疗效,我科对48例老年患者采用了该手术,并与中青年患者的疗效作了对比,现将结果报告如下。

1 资料与方法

1.1 临床资料 老年组:共48例,其中男28例,女20例,年龄为60~79岁,平均为69.4岁,病程在2年~35年,初次手术治疗39例,再次手术治疗9例;根据1999年海口会议标准[1]进行慢性鼻窦炎鼻息肉分型分期:Ⅰ型1期7例,Ⅰ型2期4例, Ⅰ型1期3例,Ⅱ型1期5例,Ⅱ型2期11例,Ⅱ型3期12例,Ⅲ型6例;完善相关检查示有4例糖尿病患者和6例度高血患者,于术前清了相关科室会诊,处理后得到控制;中青年组:共44例,其中男26例,女19例,年龄为18~59岁,平均为35.4岁,病
