探讨心率对128层螺旋CT冠脉血管成像图像质量的影响

【摘要】 目的：探讨心率对128层螺旋CT冠脉血管成像质量的影响。方法：选取2017年5-11月于本院接受冠脉CTA检查的58例冠脉疾病患者作为研究对象，所有患者均为临床怀疑冠心病而自愿接受冠脉CTA检查，记录58例患者检查前及检查过程中的心率变化情况，评估患者冠脉血管成像对冠脉的显示能力，分析心率对冠脉CTA图像质量以及管腔狭窄程度评估准确性的影响。结果：58例患者检查后冠脉CTA图像质量分级为Ⅰ级45例

（77.59%），Ⅱ级12例（20.69%），Ⅲ级1例（1.72%），其中图像质量I级患者中39例患者平均心率低于60次/min，Ⅲ级者存在心律不齐且心率变化超过10次/min。重建管理中发现40例患者在75%的相位窗重建中冠脉显示效果最为理想。结论：在128层冠脉CTA检查中患者心率、心律越低则CTA图像质量越高，并且在75%的相位窗重建中能获取最佳冠脉显示效果，因此128层冠脉CTA对心血管疾病的诊断具有较高的敏感性与准确性。 【关键词】 心率; 64排128层螺旋CT; CT冠脉血管成像; 图像质量; 应用价值

【Abstract】 Objective：To investigate the effect of heart rate on 128-slice spiral CT coronary angiography.Method：58 patients with coronary artery disease underwent coronary CTA examination in our hospital were selected from May to November 2017.All patients voluntarily underwent

coronary CTA examination for suspected coronary artery disease.Heart rate changes were recorded in 58 patients before and during the examination.The ability of coronary angiography to display coronary artery was evaluated.The effect of heart rate on the image quality of coronary CTA and the accuracy of evaluating the degree of stenosis were analyzed.Result：There were 45 cases（77.59%）of grade Ⅰ，12 cases（20.69%）of grade Ⅱ and 1 case（1.72%）of grade Ⅲ.Among the 39 patients with grade Ⅰ，the average heart rate was lower than 60 times/min and the heart rate changed more than 10 times /min.In reconstruction management，40 patients were found to be the most effective in 75% phase window reconstruction.Conclusion：The lower the heart rate and the lower the heart rhythm，the higher the CTA image quality in 128-slice coronary CTA examination，and the best coronary display effect can be obtained in 75% phase window reconstruction.Therefore，the 128 slice coronary CTA has high sensitivity and accuracy in the diagnosis of cardiovascular diseases.
